Floss Threader vs. Interdental Brush vs. Water Flosser With Braces: Five Criteria, Priced 23 August 2026

TL;DR — In people wearing fixed braces, the head-to-head trials that exist compare floss against a water flosser, and they cannot separate the two; interdental brushes have never been tested head-to-head against either, which is not the same as being equal to them. A two-year course costs $58 to $211 at US retailers, a spread driven mainly by which disposable format is bought rather than which method is chosen. The differences that do hold up are geometric (what reaches under the archwire) and logistical (how many single-use units end up in the bin).

On the criterion most people ask about — plaque reduction — the head-to-head evidence in people actually wearing fixed braces covers only two of the three methods. A 2022 randomized split-mouth trial in 34 orthodontic patients found super floss and a water flosser "equally effective" for plaque removal overall, with a small edge for the water flosser on distal molar surfaces (p = 0.033), and a 2024 randomized trial in 30 fixed-appliance patients reported a small water-jet advantage that was not statistically significant. Interdental brushes sit outside that comparison entirely. On cost, our own price check at Target, DentalStores.com, Waterpik's own store and three brand sites on 23 August 2026 puts a two-year course of treatment at $58 to $211 in the US — a spread driven almost entirely by which disposable format is bought, not by which of the three methods is chosen.

What does separate them is geometry and logistics. Below, each method is scored on five criteria before any verdict: does it get under the archwire, how long does a session take, what is the evidence for plaque reduction, how many disposable units does a year produce, and what does a year cost.

Pricing method

We read twelve US listings at six named sites — Target, DentalStores.com, Waterpik's own store, Sunstar, Dental Lace and better-objects.com — on the single date given above. Eight of those listings feed the cost table in section 5; one, a GUM Proxabrush pack, was dropped for the reason given there; the remaining three appear in the reusable-holder section. Annual figures assume one use per day for 365 days, which follows the American Dental Association's recommendation to clean between the teeth once daily. Prices exclude tax, shipping and promotions, and at least one of the listings showed an active multibuy offer on the day. Interdental brush replacement intervals are stated as a range because manufacturers do not publish a fixed lifetime — TePe's own guidance is simply to change the brush when the bristles wear and start to collapse. No weights are published for any of these products, so waste is counted in units, not grams.

1. Reaching under the archwire

This is the only criterion where the three methods are genuinely not equivalent, and it is a matter of physical geometry rather than technique. The barrier is documented in an unexpected place: a systematic review protocol on cleaning aids for fixed braces had to drop the Gingival Bleeding Index from its own methodology because the index requires passing floss interdentally, and the archwire gets in the way. Researchers changed their study design around the obstacle.

The American Association of Orthodontists states the requirement functionally in its published guidance for patients. Its on-the-go kit names a floss threader or pre-threaded floss as the tool for cleaning around the archwire and tight spaces, and describes reaching food stuck "between the archwire and teeth" — the source's own wording, and the wording the whole geometry argument here rests on. Interdental brushes clear the same bar from the other direction: NHS patient guidance lists using one between the teeth and underneath the braces every day.

Water flossers sit differently in the guidance rather than in the physics. The ADA's own patient page lists braces among the reasons people "might try" a water flosser, and puts the device on a par with string floss for food removal: "The water, like traditional floss, removes food from between teeth." What that page does not do is describe a cleaning surface being passed under the archwire, as the AAO and NHS pages do for the other two. The silence is not a disqualification. It does mean the archwire question is answered explicitly for two of the three methods and left open for the third, and that is worth holding on to when reading the trial results below.

2. Time per session

We could not find a published, measured time-per-session figure for any of the three methods in a fixed-appliance population. That is a real gap, and it is worth naming rather than filling with a guess.

The closest thing to a number is a clinic estimate: one US orthodontic chain states that threading a full mouth takes about 10–20 minutes when starting out and around 10 minutes with practice. That is an experience-based figure with no measurement behind it. Manufacturers of pre-shaped orthodontic flossers claim one to two minutes; those claims carry no data either. The only properly measured flossing times we found — 106 seconds with a handled system versus 145 seconds with string floss — come from a 40-child trial in participants who did not have braces, so the numbers do not transfer.

Best hypothesis, marked as such: the time gap between threading and the two non-threading methods is the largest single driver of whether a routine survives a two-year treatment, precisely because it is per-gap rather than per-session. We have no data confirming it.

3. Evidence for plaque reduction

The honest summary is that the evidence is thin everywhere, and thinnest exactly where people assume it is strongest.

The Cochrane review of interdental cleaning — 35 trials, 3,929 adults — concluded that flossing or interdental brushing on top of toothbrushing may reduce gingivitis or plaque, but graded the evidence as low to very low-certainty with effects that may not be clinically important.

Narrowing to fixed appliances does not sharpen the picture. A 2026 systematic review of interdental brushes in orthodontic patients pooled six heterogeneous trials in 442 patients and reported only tentative indications of benefit from low-certainty evidence; its comparisons are against toothbrushing alone or control, not against floss or a water flosser, so the brush has no head-to-head result in this population at all. For water flossers, a 2026 meta-analysis in the International Journal of Dental Hygiene concluded that adding one to toothbrushing does not outperform dental floss on plaque or bleeding indices in orthodontic patients (SMD −0.88, 95% CI −2.25 to 0.50, p = 0.10). The longest trial on the question — 56 weeks, 40 fixed-appliance patients — found no evidence of benefit from a water flosser added to manual brushing, though it was stopped early because of COVID-19 with about 85% of the planned data collected.

Why anyone bothers is not in dispute: a meta-analysis of 57 studies covering 9,101 patients put white spot lesion prevalence at 55.1% among orthodontic patients versus 29.1% among untreated people. The stakes are real; the tool ranking is not established. Anything involving bleeding, pain or a loose bracket is a question for a dentist or orthodontist, not for a comparison table.

4. Waste per year

At one use per day, the threader lane produces 365 single-use items a year plus their packaging — for pre-threaded floss, that is 365 threaders across roughly 73 sleeves and about 12 boxes. Pre-shaped orthodontic flossers produce 365 plastic handles. Interdental brushes produce roughly 37 to 52 units, each combining nylon bristles, nylon-coated metal wire and a plastic handle, a mix that in practice rules out recycling. A water flosser produces four replacement tips a year — Waterpik's own guidance is a three-month replacement interval for bristled tips, which covers the orthodontic tip — plus one appliance at end of life, with no published figure for what that weighs as e-waste.

The nearest peer-reviewed carbon comparison is a life-cycle assessment of eight interdental cleaning aids over five years of daily use, which found floss picks had the largest environmental footprint in 13 of 16 impact categories at 11.42 kg CO2e over five years, against 2.11 for a standard interdental brush and 1.38 for a replaceable-head brush. Two caveats: that study did not test threaders, orthodontic flossers or water flossers at all, and floss picks are only a proxy for orthodontic flossers, not the same product. Over a 24-month treatment the totals are: bulk pre-cut super floss $58, interdental brushes $80–$114, water flosser $100–$120, orthodontic flossers $128–$211. Three of those four routes land between $58 and $120; the pre-shaped orthodontic flossers are the outlier, and the gap between the cheapest and the dearest route is roughly the price of the treatment's first month of retainers. One further product could not be costed per unit: a GUM Proxabrush pack listed at $16.99 at Target gave contradictory quantities on the same page — the title says 10 count, the specification says 40 — so we have left it out rather than publish a per-brush figure we cannot stand behind.

Weak spots in each method

A water flosser is the weakest first purchase for anyone whose problem is the archwire specifically: it is the most expensive year-one option, and the longest and largest trials in braces patients did not find it superior to floss.

For interdental brushes, tool choice follows interdental anatomy rather than preference. The ADA's guidance indicates floss for tight interdental spaces and interdental brushes for more open ones, and brush sizing is not a one-size decision — which is a conversation for the clinic rather than for a comparison table.

Floss with a threader is the route most likely to be abandoned. A routine that takes around ten minutes per session — a clinic's estimate, not a measurement — and a fresh threading at every gap asks the most of a two-year treatment, and an abandoned method has an evidence base of zero.
